ID and address proof of the original author / owner is the basic requirement for the registration, it is mainly for the clarification of your exact name and address proof and communication address must be enclosed.
Copies of Work
Every application for registration should be accompanied by two identical copies of the work.
Power of Attorney
It is an instrument of authorization to represent the act behalf of another person. An legal right given to the person by POA.
NOC
If the application is submitted by a person other than the owner No objection certificate must be enclosed by the owner in his favour along with the application.
SoP
Statement of Particulars details have to be mentioned as per the column of the application with the bonafide , applicant should also sign the Statement of particular and should mention the date and place.

Statement of further particulars in which the details regarding the translation, adaptation should be filed, applicant should also sign the Statement of Further particular and should mention the date and place.
